Our New Patient Process

  • 1. Register as a patient

    Register for your personal patient portal - only entering the required items for now - creating access for you to schedule your visit. If you need help, please feel free to call us at 801-823-2368.

  • 2. Add a valid card

    Once in the portal, select the “invoices & payment” icon, and add a valid credit card held securely on file, required to schedule an appointment. You will not be charged until we call to confirm your appointment and reserve it with a refundable deposit (>48hrs before)

  • 3. Make appointment

    Next, select the “appointments” icon, select Dr. Yorgason as your provider, choose in-person or virtual visit and then reserve the date and time that works best for you. Please call 801-823-2368 for any questions.

  • Prepare for your visit

    Now that you are a registered patient, and have an appointment, you can prepare for your appointment by completing the required items with the RED alert in each section of the portal prior to your visit.
